Dear Patient,
We would like to take a moment to welcome you to our office. In order to make our appointments as comfortable as possible, we have provided some information about our office policies and downloadable patient information forms.
Our office hours are Monday through Friday, 8:30 to 5:00 with lunch daily from 12:30 to 1:30. A physician is on call during the nights and weekends for EMERGENCIES ONLY. Dr. Goldman, Dr. Pitarys and Dr. Chadda all have admitting hospital privileges at Bayonet Point Hospital, Community Hospital and North Bay Hospital.
Dr. Goldman, Dr. Pitarys and Dr. Chadda share on-call coverage and hospital coverage. If you have an emergency, either call 911 or go straight to the emergency room. The emergency room physician you see will call the on-call physician once you are evaluated in the emergency room.
You should bring an accurate list of all medications with you on every office visit and to the emergency room / hospital. Prescription refills should be asked for when the medical assistant is taking your vitals, prior to the physician coming in. Please do not wait until your prescription runs out to get a refill. You must allow the office staff 3 to 5 days for all prescription renewals. No prescriptions will be given out or called into the pharmacy if you have not been seen in the office for at least 6 to 12 months, depending on your medical history. If you require an authorization / referral, please allow 2 to 5 days for the office to obtain the referral.
All deductibles, co-pays and office procedures not covered by your insurance must be paid at the time of service. If you are on an insurance that requires you to pay a co-payment, it must be paid on the day you are seen in the office. If you have any questions regarding billing, please contact the office staff for the billing company’s phone number.
If you should have a change in your address or your insurance companies, please notify our staff at once, so we can update our records.
