Let us take this opportunity to welcome you to our office! To make your appointments as simple and comfortable as possible, we have provided some information about our policies and downloadable patient information forms that you can complete prior to your visit.

What You Should Know About Us

Dr. Goldman, Dr. Pitarys, all share on-call duties and hospital coverage. If you have any type of emergency, we insist that you either call 911 or go straight to your local hospital's emergency room. The emergency room physician there will call our on-call physician once your condition has been evaluated.

 

We also ask that you bring an accurate list of all medications you are taking with you on every visit to our office and to the emergency room or hospital. Prescription refills should always be requested when our medical assistants are taking your vitals and prior to the physician coming in. Please do not wait to get your medication refilled after your run out of medicine. Our office staff must be allowed 3 - 5 days for all prescription renewals to be filled. Prescriptions will not be written or called into the pharmacy if you have not seen a physician in our office for at least 6 - 12 months, depending on your medical history. If you require an authorization or referral, you must allow 2 - 5 days for our office to obtain the referral for you.

 

All deductibles, co-pays and office procedures that are not covered by your insurance must be paid at the time of your appointment. If you have any questions regarding billing for a particular visit, please contact our office staff for the billing company’s contact information.

 

If you should have any changes to your address or insurance coverage, please notify our staff at once. That way we can update our records immediately to avoid any confusion during your appointment.
